By using this website, you AGREE to the following terms and conditions:

• that you meet the eligibility requirements as outlined on this website, and, as set forth by the Arizona Department of Health Services, Arizona Department of Transportation and all applicable local, state, and federal laws;
• that you have read, understand and agree to our Privacy Policy;
• that you have read, understand and agree to our Refund Policy;
• that the person on the corresponding violation being addressed is the person participating in the program;
• that you will not dispute or file a chargeback with your card issuing bank.

If participating in DUI Education and Treatment, as you will be enrolling in an Arizona Department of Health Services licensed program, all policies will be explained in detail to you by our staff, and where applicable, will require your in-person or electronic signature prior to engaging in services.
